I do not file insurance claims or have contracts with any insurance plans. Generally speaking, at the pre-testing meeting the total fee is established.  If an evaluation is to be completed 50 percent is due prior to the beginning of testing and the remainder is due at the post-testing consult.

Insurance Claims

Although I don't file insurance claims, I will assist you in filing your insurance claim by providing the statements and paperwork required by your insurance company.  However, I am not a party to your contract with your insurance company.  My only involvement would be as a third party to assist you in processing your claim and receiving the maximum reimbursement your policy allows.  Insurance reimbursement should go directly to you.  Contact your insurance company to find out what reimbursement you can expect from them for an out of network provider.

Follow-up fees

The evaluation charges cover whatever work is necessary to complete the evaluation, including review of records, phone calls, pre and post testing conferences, testing and the written report.  Follow-up meetings with school personnel, attorneys, or administrative law judges is sometimes necessary after the report is released.  If such follow-up work is requested, the standard charge will be $200.00 an hour.

At least 24 hours notice is required for cancellations.  A $175 fee will be charged for cancellations with less than 24 hours notice.
